Twin County Regional Healthcare has been notified that Twin County Home Health, located at 104 Cranberry Road, Suite 100B, Galax, VA 24333, discontinued operations effective September 4, 2026.
Twin County Home Health has operated under the Twin County name through a joint venture between LifePoint Health and LHC Group. While affiliated through this partnership, the agency has been independently managed and operated by the joint venture.
Representatives of Twin County Home Health are working directly with current patients, caregivers, physicians, and referral sources regarding care transitions and continuity of services. Patients with questions regarding home health services may contact the agency at 276-236-7935.
