Pre-Registration Guide

Please review the following information before continuing.



What is Pre-Registration?

Pre-registration allows you to complete your admission paperwork before the day of your procedure. This will save you time on the day of your visit. Usually, patients come in to pre-register as they leave the doctor's office.


How to Pre-Register:

Since all doctor's offices are not in close vicinity to the hospital, we offer several ways to
pre-register.

» In Person:

- For X-ray or Lab procedures, go to the Outpatient Imaging Entrance.
- For Inpatient Admissions or Outpatient Surgery, go the Admissions Entrance.

» By Telephone:

- Call 236-1640 for Admissions or Outpatient Surgery visits.
- Call 236-1642 or X-ray or Lab procedures.


Directions:

For Inpatient Admissions or Outpatient Surgery:

As you drive up to the hospital, veer right and park in the Admission/OP Surgery parking lot.

For X-rays or lab work:

Veer left go past the main entrance and Emergency entrance to the entrance marked "Outpatient Imaging".


When you pre-register, please be sure to have:

  1. Your doctor's order
  2. Your insurance card
  3. Your driver's license
  4. Any co-pay or deductible amounts (these will be collected at the time of pre-admission)

When will you know the results of your test?

The results of your test will be sent to your physician's office within 24 hours.


Reminders:

You may be asked to remove your jewelry for some x-ray procedures. Please consider leaving your jewelry and valuables at home.

On the day of your procedure, please use the phone in the reception area to call if you wait over 20 minutes. Patients in the reception are receiving a different test may be seen before you.
