Education/  Experience:	Masters Degree from an accredited school of social work. Minimum of two years experience in a health related field. Clinical experience in varied multidisciplinary settings.
Qualifications:	All candidates will: display a professional appearance and a positive attitude; be reliable; provide excellent customer service including positive communication skills; possess a strong work ethic; honor confidentiality; have a good attendance record; work professionally with all department staff, TCRH employees and guests.
Description:	Responsible for assisting patient/family in meeting the social, emotional, and economic needs in relation to the impact of a terminal illness; taking referrals and coordinating intakes; assisting in coordination of Hospice related programs and fundraisers; assisting in promoting awareness of hospice/services offered to the surrounding community.
